Consent to Personal Data Processing (GDPR)

You hereby grant consent to Karlin Dent s.r.o., with its registered office at
Křižíkova 331/87, Prague, ID No.: 23302259, registered in the Commercial
Register maintained by the Municipal Court in Prague (hereinafter referred to as the “Controller”), to process the personal data listed below in accordance with
Regulation (EU) 2016/679 of the European Parliament and of the Council (GDPR).

Processed Personal Data

The Controller may process, in particular, the following personal data:

  • name and surname
  • e-mail address
  • phone number
  • content of the message sent via the contact or order form
  • technical data (IP address, cookies)

Purpose of Personal Data Processing

Personal data is processed for the purpose of:

  • scheduling a patient for dental treatment,
  • contacting the patient back,
  • communicating with persons interested in the clinic’s services,
  • fulfilling the legal obligations of the Controller.

Data Retention Period

Personal data will be retained for a period of 3 years, or for the period established by applicable legal regulations.

Cookies

The Controller’s website uses cookies for the purpose of:

  • ensuring proper website functioning,
  • analytical evaluation of traffic (Google Analytics),
  • marketing and remarketing purposes (Meta / Facebook Pixel).

Cookies can be:

  • technical (necessary),
  • analytical,
  • marketing.

By using the website and granting consent, you agree to the use of cookies.

Personal Data Processors

Personal data processing is carried out by the Controller. However, personal data may also be processed by the following processors:

  • web hosting provider,
  • web / IT agency,
  • providers of analytical and marketing tools (Google, Meta).

The Controller currently does not use other personal data processors.

Rights of the Data Subject

You acknowledge that under the GDPR you have the right to:

  • withdraw your consent at any time,
  • request information on what personal data is being processed,
  • request access to your personal data and their correction,
  • request restriction of processing,
  • request deletion of personal data,
  • to data portability,
  • lodge a complaint with the Office for Personal Data Protection.
